Fitness For Work Declaration Form

At Gunther Civil, we ensure our employees are fit for work by enforcing:

• At every Pre-Start Meeting, you must present for an alcohol breath test which will be carried out in accordance with AS/NZS 3547:1997 Breath Alcohol Testing Devices for Personal Use.
• If your result is 0.00, it is deemed a “pass” and you can return to work as normal.
• If your result is greater than 0.00, you must be re-tested after 15 minutes but no greater than 60 minutes from the original test. The result from the re-test will be your final result.
• If the final result is a “pass” you can return to work as normal, with no disciplinary action or record.
• If your final result is a “fail” you will be transported to your place of residence at your own cost. If you are a sub-contractor you will be required to leave site.
• You will also be issued a verbal warning if this is your first instance of disciplinary action.
• If this is your second instance you will be issued a written warning.
• If this is your third instance, your employment will be terminated.
• The result from every alcohol breath test will be recorded, however only results deemed a “fail” will be noted in the Employee File. All results are kept confidential.
• Failure to submit to an alcohol breath test will automatically be deemed a “fail.
• Employees who have been deemed a “fail” will not be authorized to drive themselves to their place of residence. If they do drive after their Supervisor has directed them not to, they will be at breach of the law and cannot make a claim to Work Cover if an incident occurs.
• If you are involved in an incident, you will be required to present to the nearest medical facility for a mandatory Drug & Alcohol test. You will be accompanied by a Gunther Civil Supervisor who will be responsible for the complete management of this process.
• If your test is positive, you will be subject to disciplinary action.
